VLED

VERY LOW ENERGY DIET .. abrv. VLED. @ VERLY LOW CALORIE DIET

Got a chance to attend a talk by Dr Anthony R Leeds, at the Faculty of Medicine and Health Science, UPM.

VLED is a total diet replacement with an energy content between 450 kcal to 800 kcal, in the form of fortified liquid meals which provide vitamin, mineral, trace elements, essential fatty acieds requirement on a daily basis. This source of food usually administered over 12 - 16 weeks period, however recent guideline from UK National Institute for Clinical Excellence indicates this should be limited to 12 weeks.

This idea of VLED far back in 1920. It has faced an initial problem with the use of low quality and insufficient protein and inadequate vitamin & mineral supply. However has been corrected in early 1970s. The approach of VLED base on theory, where once body achieved a greater energy deficit than a low energy diet, this will result in greater rate of weight loss. Due to energy requirement is differ from each indivual rate of weight loss ~1.5 kg/week and ~2 kg/week for women and men respectively.

Follow up study for a minimum a year shown a significantly and dramatically drop in body weight after 6 month of VLED diet. However, this drop started to gain back another 10 kg and reach the same weight as those having other program (diet+exercise, drug therapy). The end point, VLED resulted the same as diet+exercise and drug therapy.

This regimen of diet is definately for short term as in
-preparation for bariatric and also other surgery
-where a clinical conditions limit control of energy intake pr energy utilisation such as in osteoarthritis.
-where best current therapeutic practice has failed to achieve desired outcomes such as in type 2 diabetes mellitus.

Personally, there is a doubt about this approach in T2DM management as this VLED kind of neglecting the actual cause of T2DM; physical inactivity and visceral obesity. Myself I'm still so much believe the best approach is lifestyle modification (diet and exercise). However, as the speaker also stated .. this VLED is for short term usage hence this regimen of diet might be able to give somehow an encouragement and motivation to those trying to reduce weight. Have to make sure, any patient undergoing this VLED is well informed about the weight gain after the significant loss.

Not to forget, the weight loss is due to very low energy intake hence body might switch its metabolic condition into hunger state, resulted in lower BMR., which is not so good. In any condition, exercise, need to be incorporated as muscle need to be activated. Weight loss due to VLED is a general weight loss meaning to say, body will loose both lean and fat mass. Loosing lean mass (muscular part) could lead to sarcopenia. Again, contradict to what shld be achieved in T2DM management.

Gym Attire

Wanna put this entry for such long time .. just could not get the feel to type it on...

Talking about getting fit and healthy ... definately ... one of the component is . doing appropriate exercise. :-). For this purpose Fly, Marul, Rashid, Anson and me .. join so called BodyStep class. We try out best not to miss our fav instructor ... namely .. Aunty Arjiete. One thing about this Aunty, she hardly inform the management if she could not make it for the class so, the management have to call any available instructor to replace her.

For last few week, this particular Aunty, again could not make it... well holidaying in Bangkok and also coz her mom passed away (from God we came ... to Him we return). That particular week, the momment I saw the replacement instructor, straight away decided not to join the class and informed my buddies too. Ended up we had early dinner .. haha gaining fat instead of burning fat.

Besides us, there are two Ausie ladies whom I think a die hard fan to Aunty Arjiete as well.

Jeng jeng jeng ... the main story is coming. (SUPRISE!!) hahahaha

One of them was in the class. Hmmmm

Two week after the class I got the chance to talk to her .... she was complaining about the replacement instructor and bit upste coz I didnt inform her about that. The ULTIMATE complain from her was .... the instructor wearing super short pants and the moment the instructor did the side strecth ... his ball was out from his short !! Gosh ... disgusting!!

Susan : "Fitri, I got more that what I'm suppose to get" hahahaha still remember this phrase. I'm so sorry Susan, unintentionally let you join the class. Hahahaha

Back to my entry title .... please please please be in proper attire for doing exercise.

Getting Fit ......Ethically

It has been proven by abundance of scientific researches about the beneficial of exercise, regardless the type of exercise namely strength training, aerobic (mostly people just say it cardio), and flexibility training.

To give some example for each type, strength or resistance training can be included free weight training or weight training using the weight machine or well known patterned group exercise for this type is BodyPump from LessMill training system.

Aerobic exercise such as running (indoor or outdoor), swimming, cycling (stationary or not), or those attending gym surely familiar with BodyStep, BodyCombat and BodyAttack or for more challenge gym member may join rpm class, which is stationary cycling in a group lead by an instructor.

As for flexibility, individual may join YOGA, Pilates, Hattha YOGA, Power Yoga, BodyBallance and few more of simply do a proper stretching. All these three type of exercise are needed to ensure each component of physical fitness is optimum.

It is amazing to see the growth of fitness center or we simply call it gym. It becomes a lifestyle especially for younger working class. Joining a gym nowadays not only for the purpose of getting fit but also is a way to socialize and to get some relaxation after a hectic work in office. Well, some may get more than this and I’m not going to mention it here. Who tastes the honey will know how sweet it is. (to replace, siapa makan cili dia yang pedas, Who eats chilli will know how hot it is as this normally refers to something unwanted or unbeneficial effect)

To name some of it, Fitness First, TrueFitness, FitnessNetwork and California Fitness. I must say, Ministry of Youth and Sports also play an important role in this changes by providing a public gym as we could see it in Desa Pandan and also in Titiwangsa Park. With the cost much lower, exerciser can simply get exercise equipments by visiting the public gym.

Talking about joining, attending and working out in the gym, few things that proved our physical development is much better as compared to mind attitude changes. Some may realize and some may not, but it is happening. Shall I name it as gym jerk?? Will it be too much??

The sweat giver – this type of gym members always annoy me. They simply leave the exercise machine with their sweat on it. I’m sure nobody….. NOBODY wish to share their sweat. Please and please bring the small towel with you and wipe the machine, if you (guys) feel carrying the towel make you sissy, feminine or pondan (pardon my vocabulary) go get the towel provided and wipe your manly sweat (if it is really a man sweat!) and for this, ladies, I cant think of any excuse for not wiping your own sweat.

The odor giver – I’m not blaming God creations. Some of us were born with strong body odor. God created this individual together with the way how to encounter this problem. Please, please do not wear the same gym attire for few consecutive days.

The smelly footer - I swear to God, none will feel please when this type of members taking off their gym shoes in the changing room. Suffocated!! Intoxicated!! Again, I am not blaming them for having the problem, but knowing you are having this type of smelly feet, please take some action. Change your sock as often as you could and get two pairs of gym shoes so that you can rotate and let one of it well dried.

The free weight mister – this group they have few unpleasant attitudes. Well I’m not envy with their work out result … but the attitude really unhealthy. Few subcategories for this Free Weight Mister.

One – get almost all the dumbbells and make it like “all are mine”. Definitely you can not lift the entire dumbbell at the same time. Leave some for others.

Two – related to the number one, some do not know to return the weight back on the rack. I might understand if new member doing it, as they might not familiar with gym ethic, but it is unacceptable for gym freaks or gym goers to do the same. Ohh! Hate the most when the gym trainers doing the same, some so called PT simply leave the weight after the session! Gosh. Don’t these trainers equip with gym etiquette?

Three – lift the super heavy weight and at the end of the set, just let the weight hit the floor. Wow…. It is like telling the entire gym member you are the strongest man ever. You are so man….! Duhhh. Come on, the overload training principle is not until you can not control the weight.

Four – soundtrack maker. These Mr Free weights, I always wanna laugh at them. Lifting weight which is suitable for ladies but producing the sound like the one you could get from hardcore adult movies. Funny but annoying sometimes.

The restaurant goers – When the lounge area becomes a restaurant, some members, either consciously, subconscious or unconscious just leave the cup on the table. Come on!! The lounge is not a restaurant! None of the staff inside is a waiter/waitress.

The meeting organizer – I mentioned earlier on about joining gym is not only for exercising but it could be a way to socialize BUT not until having a long hour meeting in the middle of the exercise machine area. Saying hi and gossiping is OK (who doesn’t!) but not until blocking others.
